Bring a valid, unexpired government-issued photo ID (driver's license, state ID, or passport). For Live Scan, also bring the Request for Live Scan Service form from your requesting agency — it contains the ORI code we need to transmit your prints to the right place. For FD-258 ink cards, bring any cards your agency provided, or we can supply standard cards.
